Below please find our permissions form outlining the details needed to process your permissions request.  Please note that we do not handle domestic permissions for our University Presses.  Please visit our University Presses directly to submit a form for domestic permissions requests.

Please note that for permissions for use in academic journals or access to author archives, we also require a CV as well as a full annotated list of the requested excerpts with proper credits. These materials should be emailed as Word doc or PDF attachments with an identifying subject line to simultaneously with the completion of the form below. Please be aware that without these materials we will be unable to grant your permission and your request will be denied. 

Please note that due to a high volume of requests, we generally require 10-12 weeks to complete the permission process.
